There are a variety of reporting forms that we use to help track your student’s progress. You are welcome to create your own forms, as long as they convey the same information, but ours are available here for download.
Required forms for each grading period include the attendance record,
PE form, and grade summary with course of study.
We have a few pre-written courses of study for High School elective classes.
There is also a blank course of study sheet to help you design your own class.
We are happy to help you determine what should be required for an elective of your choice.
Please note that grades should be reported in percentage form. Please also include the scores from all major tests, quizzes, or projects from which the final grade is determined. This helps us to have a clear picture of your student’s work. Just submitting a single letter grade for the semester does not convey a sense of the work accomplished.
On all forms turned in, please be sure to include the student’s name, grade level and the school year and semester for which they apply.
Remember that we request grades to be turned in at least twice a year. If your student does not complete a full year’s curriculum in the school year, please still send us a report letting us know what they have accomplished. This is how we maintain accountability with you for your enrollment period. Student’s for whom we have no record of work completed may be dropped from our program.
